UI Mod-Non-defaults

Where can I find a mod like this or why won't it allow me to see the page?

Rez Dalnava's version of the mod is out of date - it breaks CAS. There is an updated version somewhere, but I never can remember where.
Remember that you only need it if your mouse doesn't have a scrollwheel.
