Sims can be made fat in Create a Sim, or can get fat from eating too much/not exercising. If you want your sims fatter than they can get in-game, you'll have to download custom meshes for that - new 3D shapes that are fatter than normal. To make a sim fat all the time, you'll also need a mesh-linked skintone to put them in (so they stay fat when showering/naked) and fat outfits for all clothing types.
