Why does this CAS Custom Content item look distorted?

I downloaded this D.Gray-man Forehead Crosses for CAS and tried to make a sim with it. However, when I selected it, it looked messed up on her forehead! Is there something wrong with my Sims 3? How do I fix it?

As you can see, it looks normal in the icon. But when it's on a Sim's face, it looks distorted!

Turn your texture detail on high. (And possibly sim detail on Very High. At least High.) Then view in game instead of in CAS. Is it still messed up?

Looks like your sim pressed her head against an electric fence.
