Default Shihoin's Thomas Knight's armor
I can't find a description of who made this outfit, but Shihoin's site seems to be down. Does anyone know where I can find it? Alternatively, does anyone know of any suits of armor that specifically have a high neck guard?

That's the armor that comes with the Ambitions EP, via the Time Machine. You can unlock it using the UnlockOutfits cheat.
