From the TS3 version of that thong I remember that it said there was some kind of reaction by other Sims when they see the thong sticking out. Unfortunately, this doesn't seem to be the case for this thong? Would it be possible to create something (a mod) like that?? Would be amazing!!

Hi, thanks for this item. It's 2022 now and a lot of new gameplay tags make this item harder to use. I've fixed a lot of the tags, including colours, appropriate outfit and species. You can download the updated version from my Tumblr https://puremorphinsims.tumblr.com/...-for-the-sims-4 and if this MTS version wants to upload my fix, I'll then take down my Tumblr post.
