This user has the following games installed:

Sims 2, University, Nightlife, Open for Business, Pets, Seasons, Bon Voyage, Free Time, Apartment Life
Default Blank Wants Slots (solved)
Today Angela Pleasant woke up with 1 want slot missing and 2 fear slots missing, the missing slots are blue, rerolling them ends up with the same Wants and fears and the same slots missing, upon fulfilling a want they fade away and are replaced by a blank slot, taking the sim on a date solves the problem while on the date, but when it's over the same wants and same blank slots take place again. This is the first time this happens to me, and when I searched about it on the internet people said rerolling the wants would solve it, but it isn't working for me.

Try and delete your cache files then restart your computer and try it out.

If this is the PV that came with the game then it is corrupted from the start and you may have hit the corruption sooner than later.

Blank want slots in general are a sign of neighborhood corruption - in fact, if you're playing the regular Maxis Pleasantville, that's almost certainly the reason you're seeing those. All Maxis neighborhoods are corrupted from the beginning, and it's unfixable to boot.

Of course, that doesn't mean your neighborhood is doomed. The first thing you should do is run Mootilda's HoodChecker http://modthesims.info/d/456523 and have it fix things. It'll find a huge amount of errors, but the only ones you need to worry about are those it can't fix.

This might actually fix Angela's wants. Even if it doesn't, I've had empty want slots on a few sims and they eventually went away on their own.

I'm sorry, I should've have mentioned I'm using clean fixed templates and I already used the Hoodchecker but it still happens, I'm going to try deleting the cache files now. Thank you guys for helping.
Edit: deleting the cache files didin't work.

Okay, don't panic. It may be corruption, but at least one user has had blank want slots caused by a mod conflict; and it's also possible for blank want slots to afflict a corrupted lot without infecting the rest of the neighborhood. I don't remember what the conflict was, but it was AndrewGloria who had it, so you could check in with him.

I'd back up the game and start troubleshooting. Put her on a different lot - does the problem persist? If it doesn't, move her back in to the original lot - if the problem returns, you may reasonably conclude that the lot is at fault. The lot might be corrupt, or it might just be something on the lot, so pull your downloads. And so on.

Remember, with the game backed up on an external drive, nothing you do in the test hood matters, so don't hesitate to follow up weirdass ideas. It could be a single file that your computer hiccuped during download and corrupted - nothing is too strange to happen with code this complex.

I will do my best to try to find out what is causing this, I would start the neighborhood over again but I'm worried something on my files might be causing it since I just started the neighborhood yesterday and it's already acting weird. Well, the only way to find out is trying, thank you!

Edit: Solved it using the 50% method, it was being caused by the mod adultsgosteady.

Blank wants slots can happen when the game just comes up with nothing for that particular sim. It's not necessarily a problem and they will reroll soon enough.

I think that was the mod that gave Andrew trouble. Perhaps you also don't have all EP's moveobjects? i've used the mod for a good couple of years with no issues.

I'm using the ultimate collection, so I have all the EPs, maybe the mod was conflicting with a mod on my Fixes folder, since I only used the 50% method on my Mods folder. HCDU can't find anything though.
